When a telephone request for home health services is received, it is routed to the Holy Redeemer Home Health Intake Department. At that point information is collected by telephone in accordance with agency standards. We verify the accuracy of this information by contacting the attending physician’s office, the third party payor, and if necessary, the referral source. Any personal health information about patients, clients, residents or customers is protected in compliance HIPAA Privacy Policy standards.

We do not employ cookies. A cookie is a small text file that our Web server places on a user’s computer hard drive to be a unique identifier. We do analyze our website logs and user statistics to improve the value of our site. Our logs and user statistics are not personally identifiable, however, and we make no attempt to link these logs and user statistics with individuals that view our website.

Our site includes links to other sites. If you chose to visit another site, we cannot assume responsibility for its content or information handling practices. We encourage you to review each site's privacy policy prior to entering into transactions with it.

The information collected by LifeAssess.org will be used for tracking our audience and, if requested by the user, for referral to the LifeAssess Program of Holy Redeemer Home Health and Hospice Services. Users who provide information will receive a follow up response by email or telephone, whichever is indicated by the user.

We may from time to time seek to have our site certified by independent certification organizations. Such certifications may require an audit of our website, including your Personal Information and the Aggregate Information. We will request that each such certification organization not further use or disclose Personal Information for any purpose other than auditing this website for compliance with its certification qualifications. Of course, we cannot guarantee their compliance with our requests.
