Summary

If you (or your loved) one answered “yes” to any of the Falls Risk Survey questions, you could have a high risk for falls. It is best to get help before you have a fall. If you have had one or more falls in the past month, it is very important to let your doctor know.

Your doctor can refer you to the LifeAssessSM Program at Holy Redeemer. The professionals within Holy Redeemer Home Care's interdisciplinary team provide treatment options to help minimize one's risk for falls.  Working closely with your doctor, we provide recommendations and rehabilitation based upon the results of our specially designed screening tool. 

The interdisciplinary team includes physical therapists and occupational therapists, as well as nurses specializing in rehabilitation, social workers, and home health aides. By offering intervention, we are able to help people lead more active lives at home and in the community.
