Dementia is a complicated condition that requires a team approach to care.  If you think that you or your loved one has dementia, talk to your doctor about treatment options that may be appropriate for your specific situation. Every individual is unique, and the treatment for each condition may be different.

Your doctor can refer you to the LifeAssess^SM Program at Holy Redeemer if you qualify. The professionals at Holy Redeemer Home Care provide assessment, treatment options, recommendations, and rehabilitation. 

Intervention is based upon the results of our specially designed screening tool and physician's orders.  The interdisciplinary teams include nurses, occupational, physical, and speech therapists, as well as social workers and home health aides. By offering intervention, we are able to help people continue to live at home for longer than they otherwise might.
