Self Survey for
Dementia....*
Here
are surveys to help
determine if you or your loved one has
mild
or
moderate
dementia. If you or your loved one are concerned
about any of the following, please contact your physician:
Mild
Dementia
(Can
last two to four years or longer)
Those with mild dementia often do
better if they start treatment early. Medication can
often slow down the progression of symptoms of dementia.
Treatment can help keep people as independent as possible
for as long as possible. However, irreversible
dementia does get worse over time. It is important to
know what to expect. Use this checklist to help you
recognize problems at this stage of the
disease.
