LifeAssessSM

Staying Well at Home

LifeAssessSM is a comprehensive homecare assessment, education, support, and rehabilitation program for older adults and their caregivers.

LifeAssessSM is available through Holy Redeemer Home Care in Pennsylvania and New Jersey.

Goal

Our goal is to allow at-risk seniors to live safely and comfortably at home.

 

Four Syndromes

In an attempt to improve the quality of the service that we provide to homebound adults age 85 and older, Holy Redeemer has identified four common problems that affect individuals at this age: Dementia, Depression Falls Risk, and Urinary Incontinence.

By learning more about each of these problems, you may be able to determine if you or your loved one could benefit from the LifeAssessSM Program at home.

Eligibility for services requires a physician's order, and, in some cases, verification of homebound status and insurance approval.
